Creditable Coverage Letter:
This letter is intended to communicate that the current prescription Rx coverage being offered is creditable. That means it is better than the new Medicare Part D coverage. Where it indicates 'Insert Name of Entity,' that is the name of the health carrier i.e. Capital BlueCross. Where it indicates 'Name of Plan,' that is the name of the health plan i.e. Capital BlueCross PPO Plan.

Non-Creditable Coverage Letter:
